Which makes me wonder: does each new incarnation of the Doctor celebrate their birthday (assuming they go for such a Terran custom) on the anniversary of their regeneration?

Well, as you said, if they celebrate birthdays(or regeneration days). But even if they did, would they still do that after the first couple hundred years? Then it might just come down to the individual regenerations. #3 might not, but #6 might.

You can buy candles with numbers on them. :P I still think that after a few hundred years, birthdays would kinda lose meaning. Well, When you reach a few hundred years(which should be in a year or two), let me know if you still celebrate.
