I think it IS a joke. I mean, it was Sickles' joke. His sense of humor is not mine, and I don't have the context for any of the in-jokes (nor do I especially want them). Monty Python pulled off their combination of absurdity, political humor, slapstick, and gross-out humor, because they had very good writing and comic timing. (Sometimes, of course, Monty Python failed to pull it off.) Sickles and his team of losers don't have the skills to make their jokes funny, but they're still obviously jokes.
Yes, the portrayal of "first aid" is ridiculous. I don't think for a second that's because the writer believes human bodies work that way, or believes first aid doesn't matter. I think it's a joke, even if I don't think it's funny.
